Auszug<\/strong>
\nEine Hormonresistenz des Prostatakarzinoms wird unterstellt, wenn trotz Behandlung mit Antiandrogenen und\/oder LHRH-Analoga der PSA-Wert ansteigt. Eine \u00dcberpr\u00fcfung des Testosteronspiegels, eines potenten Wachstumsfaktors f\u00fcr das Prostatakarzinom, ergibt bei Hormonresistenz regelm\u00e4\u00dfig einen dem Kastrationsniveau entsprechenden Blutspiegel. Sollte der Testosteronspiegel erh\u00f6ht messbar sein, und das ist etwa bei zehn Prozent der Patienten in dieser Situation nachweisbar, lohnen Ma\u00dfnahmen, den Testosteronspiegel abzusenken und so die Hormonresistenz zu durchbrechen. Wenn das Prostatakarzinom nicht mehr auf Hormonbehandlung anspricht, stellt sich die Frage nach weiteren Behandlungsm\u00f6glichkeiten.<\/p>\n

Welche Rolle kommt hier der Chemotherapie zu?<\/strong>
\nDie bereits 1985 ver\u00f6ffentlichten Ergebnisse von 17 Studien mit insgesamt 1.500 Patienten zeigten, dass 4,5 Prozent der Patienten objektiv auf eine Chemotherapie ansprachen. In einer 1993 ver\u00f6ffentlichten \u00dcbersicht von 26 Studien ist eine Ansprechrate von 8,7 Prozent dokumentiert. Im Jahre 1996 wurde erstmals in einer kontrollierten, so genannten zweiarmigen Studie gezeigt, dass das Chemotherapeutikum Mitoxantron zusammen mit Cortison einer alleinigen Cortison-Therapie hinsichtlich der progressionsfreien Zeit \u00fcberlegen war. Die mittlere \u00dcberlebenszeit blieb allerdings unver\u00e4ndert.<\/p>\n
